"Att skylla på generna" verkar faktiskt hjälpa

The study is among the first to show the value of genetic counselling for psychiatric illnesses, and researchers say that the service should be made more available to people suffering from mental illness. Genetic counselling helps patients understand the cause of their illness, the genetic component and how they can protect their mental health going forward.

"Traditionally genetic counselling is applied to conditions that are caused entirely by genes like Huntington's disease," said Jehannine Austin, a Canada Research Chair in Translational Psychiatric genomics and associate professor at UBC. "Psychiatric disorders arise because of a combination of genes and experience and our study shows that genetic counselling is just as valuable for these patients."

Om två synsätt som läkare kan ha inom psykiatrin

Attitude 1 says that patients know what they want but not necessarily how to get it, and psychiatrists are there to advise them. So a patient might say “I want to stop being depressed”, and their psychiatrist might recommend them an antidepressant drug, or a therapy that works against depression. This is nice and straightforward and tends to make patients very happy.

Attitude 2 says that people are complicated. Sometimes this complexity makes them mentally ill, and sometimes it makes them come to psychiatrists and ask for help, but there’s no guarantee that the thing that they’re asking about is actually the problem. In order to solve the problem, you need to unravel the complexity, and that might involve not giving the patient what they want, or giving them things they don’t want. This is not straightforward and requires some justification, so let me give a few cases where Attitude 2 seems to me obviously correct.

NIMH skrev:Kane and his colleagues trained clinical staff at real-world clinics around the country to use a coordinated specialty care treatment program called NAVIGATE. The program featured a team of specialists who worked with each client to create a personalized treatment plan. The specialists offered recovery-oriented psychotherapy, low doses of antipsychotic medications, family education and support, case management, and work or education support, depending on the individual’s needs and preferences. The treatment was guided by shared decision making between the client and team. In addition, the treatment involved family members as much as possible.

Nikotin som ett psykofarmaka?

Jag hatar nikotinberoende, jag började själv röka när jag gick i gymnasiet och har slutat flera gånger, nu har jag gått över till att använda e-cigaretter som troligtvis är mindre skadliga men samtidigt innehåller an massa tillsatser som kan vara ohälsosamma. Det är dock svårt att hitta något som är lika ohälsosamt som en cigarett. Men det som gör dessa saker ohälsosamma är främst tillsatser och skador på grund av att man andas in varm rök (edit: tydligen är detta inte på grund av den varma röken utan på grund av kemikalier i röken) som gör att man får ärr i sina lungor och bränner bort hår i halsen som är viktiga för att transportera slem och andra immunrelaterade saker.

När jag var 21 slutade jag röka och var helt nikotinfri i tre år sedan blev jag deprimerad och började röka för att det kändes som att jag mådde bättre, jag kände mig vaknare och kunde fokusera mer.

För några år sedan läste jag en artikel som hade undersökt förhållandet mellan rökning och mental ohälsa, 60% procent av de med någon form av mental ohälsa rökte också regelbundet.

När man läser om vad som kan hålla kvar en i beroendet av nikotin så handlar det ofta om att nikotin inledningsvis hjälper mot stress och koncentrationsproblem medan nikotin förvärrar dessa saker så fort man får abstinens vilket kan vara ganska snart. Vissa uppskattar ett rökare spenderar den mesta tiden med abstinens. Detta är för att nikotin elimineras ganska fort i kroppen.

Så konsensus har länge varit att nikotin är mer dåligt än bra för oss, trots att substansen sades vara bra för en massa saker under tidigt och fram till ungefär halva nittonhundratalet. Men kan det vara så att nikotin faktiskt har fördelar som hjälper många med stress, vakenhet och koncentrationsproblem och att det är därför personer med psykisk sjukdom så ofta röker? Skulle det inte då vara mycket bättre om nikotin som levereras i en konstant koncentration användes och att det faktiskt klassas som ett läkemedel?

Clinical Correlates of Sustained Response to Individual Drugs Used in Naturalistic Treatment of Patients with Bipolar Disorder

Abstract
Objective

To report use and treatment success rates of medications for bipolar disorder as a function of patients’ clinical characteristics.
Method

Outpatients with bipolar illness diagnosed by SCID were rated by research assistants on the NIMH-LCM and those that had an good response for at least 6 months (much or very much improved on the CGI-BP) were considered responders (treatment “success”). Clinical characteristics associated with treatment response in the literature were examined for how often a drug was in a successful regimen when a given characteristic was either present or absent.

Results

Lithium was less successful in those with histories of rapid cycling, substance abuse, or (surprisingly) a positive parental history of mood disorders. Valproate was less successful in those with ≥ 20 prior episodes. Lamotrigine (LTG) was less successful in those with a parental history of mood disorders or in BP-I compared to BP-II disorder. Antidepressants (ADs) had low success rates, especially in those with a history of anxiety disorders. Benzodiazepines had low success rates in those with child abuse, substance use, or ≥ 20 episodes. Atypical antipsychotics were less successful in the presence of rapid cycling, ≥ 20 prior episodes, or a greater number of poor prognosis factors.

Conclusion

Success rates reflect medications used in combination with an average of two other drugs during naturalistic treatment and thus should be considered exploratory. However, the low long-term success rates of drugs (even when used in combination with others) that occurred in the presence of many very common clinical characteristics of bipolar illness speak to the need for the development of alternative treatment strategies.
