Dopaminagonist ny medicin för behandlingsresistent depression?

Pramipexole (Mirapex, Boehringer Ingelheim Pharmaceuticals, Inc), which is used to treat Parkinson's disease, can significantly improve symptoms of treatment-resistant depression, provided patients can tolerate the doses needed to achieve a therapeutic effect, new research suggests.

"These were quite treatment-refractory patients who did remarkably better on an adequately dosed dopamine agent," session co-chair Steven Hollon, PhD, Vanderbilt University, Nashville, Tennessee, told Medscape Medical News.

"The take-home message from this talk is that we should use a dopamine agent like pramipexole to deal with anhedonic treatment-refractory patients."

Stor svensk studie om faktorer som är associerade med självmord

Previous suicide attempts, recent affective episodes and recent psychiatric inpatient care are the strongest indicators that a patient with bipolar disorder may attempt suicide, show results of a large Swedish study.

This implies that clinicians should “pay attention to the risk of suicidal behaviour in bipolar patients with depressive features and more severe or unstable forms of the disorder”, write Dag Tidemalm (Karolinska Institutet, Stockholm) and co-researchers in PLOS ONE.

The researchers looked at 18 risk factors that they thought might be associated with attempted suicide in 6086 patients (60% women) registered in the Swedish National Quality Register for Bipolar Disorder between 2004 and 2011. The patients were followed up annually from 2005 to 2012, with a median follow-up period of 2.4 years.

During follow-up, there were 13 fatal and 338 nonfatal suicide attempts, and the rate of attempted suicide was significantly higher in women than men, at 6.9% versus 4.1%.

Having a previous suicide attempt at baseline was the strongest predictor of attempted suicide during follow-up, with odds ratios (ORs) of 3.93 and 4.24 in men and women, respectively.

Affective episodes in the year before baseline were associated with a 3.63-fold increased likelihood of a suicide attempt during follow-up among men and a 2.81-fold increased likelihood among women. The corresponding ORs associated with psychiatric inpatient care during the year before baseline were 3.57 and 2.68.

Having at least four depressive episodes prior to baseline approximately doubled the likelihood of a suicide attempt in both men and women, while multiple mixed episodes were associated with suicide attempts in women only (OR=1.40). No associations were observed for manic episodes in either gender.

“These results point to the fundamental importance of observing signs of depressive symptomatology and supplying adequate treatment, in the follow-up of bipolar patients”, Tidemalm et al remark.

Eating disorders and complicating social factors were also associated with suicide attempts in both men and women, but substance abuse was only a risk factor for men, and early onset of disease and having personality disorder were only significant in women.

The researchers say that the differing risk pattern between men and women is a “novel finding” that warrants further investigation.

Others variables that were investigated but showed no association with the likelihood of a suicide attempt included a family history of affective disorder, complicating somatic factors and violent behaviour.

"Progress is being made in how American adults view mental health, and the important role it plays in our everyday lives. People see connection between mental health and overall well-being, our ability to function at work and at home and how we view the world around us," said Dr. Christine Moutier, chief medical officer of the American Foundation for Suicide Prevention.

The study, published in the journal Health Affairs earlier this week, found that doctors neglect to follow up with their patients who have a depression diagnosis. They also are less likely to help depression patients manage their illnesses. Meanwhile, these doctors are more likely to engage in care strategies with patients who have chronic physical illnesses like diabetes.

The researchers used survey results from a thousand U.S. primary care facilities, taken between 2006 and 2013, to examine doctors’ strategies for treating patients with depression compared to those used to treat congestive heart failure, asthma and diabetes.

While those with physical ailments were treated with the proper protocol outlined for a chronic condition — such as doctors teaching patients about their conditions and reminding them about their specific treatment plans — those with depression didn’t receive the same level of care, as NPR reports.

A considerable dearth of products to meet patient needs means there are significant opportunities within a declining bipolar market, reports research and consulting firm GlobalData.

The report describes the current bipolar pipeline as ‘sparse’. Only two treatments – Otsuka (TYO: 4578) and Lundbeck (LUND: CO) drug Abilify Maintena (aripiprazole) and ITI-007, licensed from Bristol-Myers Squibb (NYSE: BMY) – are expected to receive regulatory approval by 2024.

GlobalData forecasts that Abilify Maintena will achieve sales of $643 million in 2024 across the eight major markets of the US, France, Germany, Italy, Spain, UK, Japan and Canada.

Yet there remains a range of unmet needs for bipolar disorder.

In particular, novel, effective treatment options are needed for bipolar depression. Drugs must have improved safety profiles and physicians must be better informed about them, the report states

It adds that the drugs should be developed specifically for bipolar disorder, rather than manufacturers targeting approval simultaneously with other indicators.

Thomas Parker, GlobalData’s senior analyst covering neurology, said: “A novel product that demonstrates efficacy in terms of reducing depressive symptoms versus placebo would likely be viewed favorably by physicians, providing its safety profile is comparable to the newer atypical antipsychotics.

“An important opportunity also exists for the development of a novel mood stabilizer with an improved safety profile that is indicated for the long-term maintenance treatment of bipolar disorder. Current mood stabilizers are associated with a variety of adverse events that can significantly impact treatment outcomes.”

The trauma of war may have an even deeper psychological toll than previously thought. A new study in the BMJ (formerly the British Medical Journal) finds that refugees are at a higher risk for schizophrenia and other psychoses.

In an analysis of 1.3 million Swedish citizens, a team of psychiatric epidemiologists found that refugees were 3.6 times more likely to develop psychotic disorders than their Swedish-born counterparts. They were also 66 percent more likely to do so than migrants who were not refugees.

The study was released today, coinciding with the fifth anniversary of the beginning of the Syrian civil war. As refugees from the conflict continue to pour into Europe, these findings highlight refugees' mental health needs and some of the ways in which current services fall short. "The burden of mental health disorders which occur in already vulnerable populations is an important humanitarian issue," says James Kirkbride, a coauthor of the study and a senior research fellow in the Division of Psychiatry at University College London.

The authors of the BMJ study believe that traumatic experiences exacerbate a person's risk of developing a psychotic disorder. They estimate that 0.13 percent of refugees in Sweden have developed a psychotic disorder. Last year, Sweden decided to accept more than 32,600 refugees. According to the study's findings, about 41 will likely develop a psychotic disorder in the years after their arrival. Though the data on children is limited, Kirkbride says it appears that people who migrate earlier in life may be more likely to experience psychosis. The authors also suspect that a difficult resettlement process could exacerbate these effects, though further study is needed to confirm this.

Emotional disorders such as anxiety and depression are still more common than psychotic disorders among refugees, just as in the general population. The National Center for PTSD estimates that up to 31 percent of refugees suffer from depression, and some studies show that as many as 86 percent suffer from post-traumatic stress disorder.

When asked whether he worried that his study's results might increase the fear of refugees, Kirkbride admits that the stigma surrounding severe mental illness abounds. However, the best response, he says, is to provide refugees with psychotic disorders with the treatment they need. "If you subscribe to the view that we have a moral duty to care for those we accept in our countries as refugees, this should extend to providing appropriate mental health care."

The research, published March 14 in the journal Proceedings of the National Academy of Sciences, involves studies of an inhibitor of soluble epoxide hydrolase in rodents. Soluble epoxide hydrolase, or sEH, is emerging as a therapeutic target that acts on a number of inflammatory or inflammation-linked diseases.

“The research in animal models of depression suggests that sEH plays a key role in modulating inflammation, which is involved in depression,” said Hammock, a distinguished professor of entomology with a joint appointment at the UCD Comprehensive Cancer Center.

“Inhibitors of sEH protect natural lipids in the brain that reduce inflammation and neuropathic pain. Thus, these inhibitors could be potential therapeutic drugs for depression.”

This research is a meta analysis of 29 previous studies which comprise 3961 people, and it is the first detailed work of its kind about what happens in the organism of people suffering from depression. It studies the imbalance between the individual increase of various oxidative stress parameters (especially malondialdehyde, a biomarker to measure the oxidative deterioration of the cell membrane) and the decrease in antioxidant substances (such as uric acid, zinc, and the superoxide dismutase enzyme).

The researchers have managed to prove that, after receiving the usual treatment against depression, the patients' malondialdehyde levels are significantly reduced, to the point that they are indistinguishable from healthy individuals. At the same time, zinc and uric acid levels increase until reaching normal levels (something that does not occur in the case of the superoxide dismutase enzyme).

Abstract

Objectives

Offspring of depressed parents are at increased risk for psychiatric disorders. Although bipolar disorder (BD) and major depressive disorder (MDD) are both found in the same families, it is not clear whether transmission to offspring of BD or MDD tends to occur from parents with the same mood disorder subtype. Our primary hypothesis was that the offspring of parents with BD would be at increased risk for BD and other comorbid disorders common to BD, such as anxiety and substance use, relative to the offspring of parents with MDD. The offspring of parents with BD versus those with MDD were also hypothesized to be at greater risk for externalizing disorders (i.e., conduct disorder, attention-deficit hyperactivity disorder, or antisocial personality disorder).
Methods

Parents (n = 320) with mood disorders and their offspring (n = 679) were studied. Adult offspring were administered the Structured Clinical Interview for DSM-IV Axis I Disorders to establish the presence of psychopathology. Offspring aged 10–18 years were assessed using the School Aged Schedule for Affective Disorders and Schizophrenia, Present and Lifetime version, and parents of children under the age of ten completed the Child Behavioral Checklist. Data were examined using Cox proportional hazard regression.
Results

There was no difference in hazard of mood disorders in the offspring of parents with BD as compared to the offspring of parents with MDD. However, a number of other parent and offspring characteristics increased the risk of mood, anxiety, externalizing, and substance use disorders in the offspring, including self-reported childhood abuse in the parent or offspring, offspring impulsive aggression, and the age at onset of parental mood disorder.
Conclusions

Mood disorders are highly familial, a finding that appears independent of whether the parent's condition is unipolar or bipolar, suggesting considerable overlap in the heritability of MDD and BD. Although parental characteristics had a limited influence on the risk of offspring psychopathology, reported childhood adversity, be it in the parent or child, is a harbinger of negative outcomes. These risk factors extend previous findings, and are consistent with diathesis–stress conceptualizations.

Nature skrev:Confronted with a life-threatening situation, hormones and neurotransmitters prep us for action. Specific stress hormones — cortisol in primates, corticosterone in most rodents — are released, some of which surge across the blood–brain barrier. Stress gets everywhere: all our cells host receptors for the hormone. “Every brain area has something happen to it,” says Kaufer. The human brain has two types of receptor for cortisol. One has a six to tenfold higher affinity for the molecule than the other, and so is activated earlier, by smaller amounts of cortisol.

The hippocampus (which is pivotal for memory) and the amygdala (the centre for emotions) contain lots of the high-affinity receptors, and are, therefore, activated by slight rises in the hormone. The frontal lobe, which is involved in executive planning and control, has only the low-affinity receptor, and is activated later, after the tide has risen. And, as Lupien and colleagues found, both memory formation and recall in adults can be influenced by cortisol1.

The existence of two receptor types means that response to stress is not linear. “The relationship between circulating stress hormone and memory is an inverted U-shape function,” Lupien explains. “Up to a certain level, stress hormones are good for your memory” — when the cortisol binds only to the high-affinity receptors, the ability to lay down and retrieve memory is enhanced. When the low-affinity receptors are activated, the relationship enters the right-hand side of the U-shape and the response shifts, she adds.

The duration of stress is also important. A transient bout of stress causes a proliferation of neural stem cells and a spike in numbers of new neurons, which take at least two weeks to mature. The brain seems to be preparing itself in case a second stressor comes calling. Chronic stress is not so beneficial. It slashes investment in new neurons, prunes the tree-like shape of existing ones, and suppresses new connections.

If stress hormones remain elevated for months or years, they can stimulate physiological changes: the hippocampus shrinks and the amygdala grows, for example. Eventually, the complex feedback system that suppresses the excess secretion of cortisol is disturbed. Once this happens, the capacity to discriminate between threat levels falls away. Either everything seems threatening (anxiety) or else nothing does (depression or burnout).

Stress medför normalt ökad aktivitet i hormonsystemet som reglerar utsöndringen av kortisol. Hyperaktivitet i hormonsystemet med höga kortisolnivåer, ett tillstånd som kallas hyperkortisolism, är vanligt förekommande vid depression. Men i andra änden av skalan finns exempel på att hög stress över lång tid kan leda till hypokortisolism.        


Det är möjligt att återkommande depressiva och/eller maniska perioder med hög ackumulerad stressbelastning med tiden leder till en utmattning av hormonsystemet. Stöd för denna hypotes sågs i gruppen med bipolära patienter där högre ålder var relaterat till lägre kortisolnivåer speciellt hos patienter som inte medicinerat med förebyggande stämningsstabiliserande medicinering. Däremot förekom ingen ökning av andelen med hypokortisolism bland de bipolära patienterna som under en stor del av livet medicinerat med den stämningsstabiliserande medicinen litium. Detta innebär enligt Martin Maripuu att den på sjukdomsförloppet gynnsamma effekten av tidigt insatt litiummedicinering delvis skulle kunna förklaras av att litium förebygger utvecklingen av hypokortisolism.

Wearing virtual-reality glasses and body sensors, the patients watch their avatars mimic their body movements, making it feel as if they are one in the same. It’s called “embodiment.”

In the study, the avatar comes across a crying child. The patient was previously given generic compassionate phrases to offer the child and was instructed to deliver them slowly and softly. The child avatar was programmed to respond positively to the patient’s kindness.

Then, in a second phase, the patient takes on the role of the child and, in the virtual reality world, sees the adult avatar they just embodied approach them and offer the same words of compassion in their own voice.

After repeating the intervention three more times over a month, several patients said it had helped them be kinder to themselves, and many said it reduced their depressive symptoms. One patient said it made her realize that “we are all human and vulnerable and it’s ok to be vulnerable as an adult,” according to the study. Others said they reflected on the experience when they were upset and it helped them be more self-compassionate.

Ny roll för dopamin i sociala beteenden hos möss

The study published today in Cell explores a group found in a particular area of the brain called the dorsal raphe nucleus.


The researchers used optogenetics to show that when this group of dopamine neurons is activated in mice this prompts the animal to interact with others. The finding could hold important clues about how isolation affects people. “Social interactions are no doubt important for our mental health and play a role in conditions such as schizophrenia and depression. Interestingly, these disorders are also associated with dysfunction of dopamine neurons,” said Ungless. “We’re less likely to be able to help with such conditions if we continue to assume that all dopamine neurons do the same thing.”

Många med ångest och depression har odiagnosticerad bipolär sjukdom

Hughes and his colleagues recruited young adults from general practices in the U.K. Around 10 percent of patients between the ages of 16 and 40 who were prescribed antidepressants for depression or anxiety were also suffering from unrecognized bipolar disorder. Younger patients and those with a history of severe episodes of depression were at a higher risk for bipolar disorder.

This research is particularly relevant as it relates to groups of people who are susceptible to the seasons, including those who suffer from SAD (and mental illnesses such as bipolar disorder and schizophrenia, which sometimes have shown seasonal fluctuations as well), a link that has recently come under contention. Last month in Clinical Psychological Science researchers from Auburn University at Montgomery published the results of a population-based study questioning whether there is any validity to linking major depression with seasonal variation.

“It’s difficult to establish seasonality,” Vandewalle says, “but here in our study we had the chance to strictly control protocol for almost five days, so I’m really confident in our results.”

Neurobiologisk analys av olika psykossjukdomar

Här är en ide som jag har sett lite här och där men som kanske föreskuggar en förändring i diagnostik och behandling när det gäller patienter som idag har schizofreni, schizoaffektiv sjukdom och bipolär sjukdom med psykotiska drag mfl. Artikeln hänvisar till en studie som genom att titta på flera biologiska faktorer sorterade patienter i tre grupper som var spridda över alla de "psykotiska" sjukdomarna. Tidigare genetiska studier har föreslagit att alla dessa sjukdomar skulle kunna ses som ett "schizofrenispektrum".

Fyra studier om korrelationen mellan positivt tänkande och depression

Det här är ett gäng studier som verkligen behövs tycker jag. Hur många gånger om dagen bombarderas vi inte med budskapet om att 'positivt tänkande' är det enda man behöver för att lyckas i livet?

Alla som har varit deprimerade har väl varit med om någon form av kommentarer om att man skulle må bättre om man bara 'ryckte upp sig' och inte var så negativ hela tiden. När detta händer så drar ni fram den här länken som är en artikel om fyra studier som har tittat på korrelation mellan positiva tankar om framtiden och depression.

Det genomgående resultatet har varit att en hög grad av positivt tänkande visar ett samband med frånvaro av depressiva symtom vid tiden för det positiva tänkandet (inget konstigt än så länge) men det fanns också ett samband mellan positiva tankar om framtiden just nu och depression i framtiden. Kolla in det.

Studierna kan inte säga om det positiva tänkandet orsakar depression men de visar iaf att positivt tänkande inte skyddar mot depression på sikt.

En trolig förklaring till resultatet är att överdrivet positivt tänkande kan vara en psykologisk överlevnadsmekanism som dämpar depressiva symtom för stunden men förvärrar den på sikt.

Den här artikeln pratar om en ambitiös studie som försökt hitta orsakssamband vid de hjärnförändringar som är associerade med depression genom att titta på barn till människor som lider av depression och därför har en ca 3 ggr så hög risk att utveckla depression i framtiden. De drar slutsatsen att hjärnförändringarna går att se innan depressionen visar sig vilket underbygger neurobiologiska modeller av depression.
